To assure safety is improved by learning from the past. To look for evidence of unsafe actions that have led - and can lead - to accidents, which are comprised of what caused the accident and the cause of the accident.

In every aircraft safety investigation, there are two truths:

The 1st truth is that there are those events that cause a safety failure and then there is the root event, The Cause of a safety failure.

The 2nd truth is that these events are never the same.
